Working Successfully with Your Fusion IFM Team

Another unique asset to working with Fusion IFM is our transparency in how we operate our practice and work with patients. You will receive a detailed outline of your treatment course during your first few visits with our team. As an introduction to our services, we have listed a variety of useful operational processes below that address how our practice works.

FUSION PATIENT PORTAL

We welcome your queries between office visits and throughout the course of your care and treatment.

Please communicate using our secure Patient Portal, which you can access after your first Fusion visit. We do our best to promptly respond to patient inquiries during business hours. NOTE: Our staff will send Portal replies on the next business day our office is open (Mon., Wed., Thur. between 9-5 pm.) We appreciate you understanding we are a small practice, so responding occasionally takes longer. Please call the office if you have not heard back from us within 72 business hours.

APPOINTMENTS

DIGITAL APPOINTMENT CONFIRMATIONS

Our scheduling system will send appointment reminders via email and text. If you do not receive the reminder, please call our office to re-confirm your date/time in our system.

TELEHEALTH FRIENDLY SERVICES

We use Zoom for telehealth visits. You will receive an email with the Zoom appointment link one day before your scheduled appointment.

MISSED APPOINTMENTS/CANCELLATION POLICY

Via our Fusion Patient Portal, please notify our office 3 business days before your appointment for cancellations or rescheduling. Cancelling in a timely manner allows us to give other patients on our waiting list the opportunity to utilize your time slot. You will not be charged if we can fill your missed appointment time. We appreciate your understanding.

Missed appointment fees:

● 50% of session cost if cancelled within 2 business days

● 75% of session cost if cancelled within 1 business day

● 100% for same-day cancellations and/or no-shows

APPOINTMENT PREPARATION

We recommend scheduling your first 3 visits no more than 5 weeks apart to build and maintain care momentum. Spacing more time between sessions will allow time to independently execute their treatment plan and reduce treatment costs.

For this reason, we created programs that offer our patients the expertise and a customized functional medicine plan that your provider outlines for you, along with the education and support of our functional medicine health coaches.

After these initial visits, we will work with you to gradually extend the time between sessions. This approach allows you to implement your personalized plan and action items while being mindful of your budget.

To support you on this journey, we offer specialized programs that include:

● Expert Guidance: A customized Integrative and Functional Medicine plan created by your provider.

● Comprehensive Education: Knowledge to empower you in managing your health.

● Dedicated Support: Provided by our Integrative & Functional Medicine health coaches.

BEFORE YOUR SESSION

For a seamless first appointment, please upload the information below to your FUSION PATIENT PORTAL at least 3 days before your appointment:

● PAST LABS & TESTING

Please upload any recent labs and/or testing to the Labs and Documents section of the patient portal.

● NEW PATIENT FORMS

You will receive an email for the patient portal access. Once you are in the patient portal, please go to the Questionnaires tab on the right-hand side and fill out everything, including the 3-day diet.

● PHARMACY PREFERENCE

Indicate your preferred pharmacy location and phone number, current supplement and medication list and upload your photo.

DURING YOUR SESSION

While we ask all our patients to take their notes during the appointment, we will follow up with anything we discussed and save new topics for the next session.

●Please share your top three health concerns or goals items that you would like to discuss/expectations so we can ensure we meet your needs during the session.

● Session Times:

o Initial Consultation – 90 mins. Follow-up meetings range from 30-90 minutes. Confirm the expected appointment time with your provider.

AFTER YOUR SESSION

Session Summary/Recommendations: You will receive a handwritten summary upon the conclusion of your in-person appointment. Approximately 5-7 business days after the appointment, you will receive an email stating that your typed and finalized visit summary is available for you to view and print out. If your appointment is virtual, you receive the finalized visit summary online. You can access all your previous session summaries under Visit Summaries in the Patient Portal.

● Quick clarifying questions about what was discussed are welcome via secure messaging on the patient portal. Please save new topics not discussed during your session for your next appointment.

PARTNERING WITH YOUR PRIMARY CARE PHYSICIAN

As a healthcare specialist, Fusion will partner with your PCP for relevant updates and lab testing data. This way, your primary care doctor will be fully updated on the status of your care with us, alleviating the need and expense of redundant testing. We require our patients to have a Primary Care Physician. Fusion Integrative and Functional Medicine is a consultation-only practice. While we do our best to accommodate more urgent matters and see you in a timely fashion, we CANNOT GUARANTEE an appointment will be available during an urgent situation or emergency.

WORKING WITH FUSION IFM, AN OUT-OF-NETWORK PROVIDER

Fusion IFM is an out-of-network provider specialist:

● Fusion Integrative and Functional Medicine is not contracted with any insurance companies. This allows us to provide personalized service and dedicated time that our patients value and is integral to holistic healthcare. Being part of an insurance network often equates to a high patient volume requirement where doctors typically must see 6-8 patients/hour. Additionally, we would not have the flexibility to offer our patients a therapeutic plan that integrates conventional medicine and alternative medicine approaches.

● The majority of our services are covered by PPO-type insurance plans as an out-of-network provider. While there is a wide range of plans and reimbursement rates, most of our patients with PPO-type insurance plans receive reimbursement for 50-80% of our service fees (after any applicable deductibles and/or co-payments have been met). HMO insurance will not typically cover our services.

● Services may apply towards qualified medical expenses if you have a Health Care Reimbursement Account (HCRA) through your employer or a Health Savings Account (HSA). Please check with your insurance company regarding the specific percentages of coverage that apply to your insurance plan, and with your employer or accountant for details of your HCRA or HSA.

INSURANCE REIMBURSEMENT

Fusion IFM is an out-of-network provider; all insurance claim and reimbursement processes are the patient’s responsibility. Payment for services is due at the time of the appointment and detailed payment receipts will be provided for each visit, which can be used for reimbursement from your insurance carrier or Health Savings Account (HSA). Fusion IFM will be happy to supply any additional documentation for reimbursement, but we do not directly interact with insurance carriers.
